number 2 is indeed your BBS username, number 3's furigana is mostly for people with kanji in their names. if you'd like to put the kana, of course, that's fine, but if you don't, the english letters should do fine. as for number 8, this is the name they'll put up if you win something. if you'd like to be known by marieke, than yes, put that down again, if you'd like to be known as something else, put that, or of course if you'd like to remain anonymous, you don't have to put anything at all. good luck~
